Cancellation Policy

Your session is reserved for you. In the event that you will be unable to keep an appointment, please notify us at least 48 hours in advance, so that someone else may utilize this time. In the absence of your notification, you will be required to pay the full cost of the missed session.

Insurance

If you are uncertain whether a copay or deductible applies to standard outpatient mental health services, please contact your insurance carrier's Member Services. They can provide the most up-to-date and accurate information regarding your policy. While we can verify policy details based on reports from Provider Services, this information may not always be accurate. If services are denied for any reason, the member may be responsible for the full session rate. Our standard rates are $200 for an intake session and $175 for a standard session. For the most reliable coverage details, we recommend reaching out to your insurance provider directly.

Privacy Notice
When using insurance, therapists are required to provide a mental health diagnosis in order for benefits to be released. This diagnosis then becomes part of the patient’s permanent medical record and may be accessible to multiple parties within the healthcare system.

For patients who prefer greater privacy and do not wish to have a mental health diagnosis documented in their medical record, paying out-of-pocket is an option. In these cases, therapists are not required to assign a diagnosis or share medical information with any third party.

How Your Information May Be Shared
The team at AVIV is committed to maintaining your privacy.
Information about you will only be shared in the following situations:

  1. Legal Requirements – If AVIV receives a subpoena or official court order, we are legally required to provide the requested information.

  2. Insurance Billing – Information necessary to process claims may be shared with your insurance provider.

  3. With Your Written/Verbal Consent – If you sign a Release of Information form or verbally allow permission, your clinician may communicate with the specified person or organization.

  4. Clinical Supervision – Client cases may be reviewed during supervision to ensure high-quality care. These discussions do not include identifying details.

  5. Mandated Reporting – All clinicians are mandated reporters. If there is suspicion of abuse, neglect, or other harm, we are required to notify the Department of Children and Families.
