Cancellation Policy
Your session is reserved for you. In the event that you will be unable to keep an appointment, please notify us at least 48 hours in advance, so that someone else may utilize this time. In the absence of your notification, you will be required to pay the full cost of the missed session.

Insurance
If you are uncertain whether a copay or deductible applies to standard outpatient mental health services, please contact your insurance carrier's Member Services. They can provide the most up-to-date and accurate information regarding your policy. While we can verify policy details based on reports from Provider Services, this information may not always be accurate. If services are denied for any reason, the member may be responsible for the full session rate. Our standard rates are $200 for an intake session and $175 for a standard session. For the most reliable coverage details, we recommend reaching out to your insurance provider directly.
Privacy Notice
When using insurance, therapists are required to provide a mental health diagnosis in order for benefits to be released. This diagnosis then becomes part of the patient’s permanent medical record and may be accessible to multiple parties within the healthcare system.
For patients who prefer greater privacy and do not wish to have a mental health diagnosis documented in their medical record, paying out-of-pocket is an option. In these cases, therapists are not required to assign a diagnosis or share medical information with any third party.
How Your Information May Be Shared
The team at AVIV is committed to maintaining your privacy.
Information about you will only be shared in the following situations:
Legal Requirements – If AVIV receives a subpoena or official court order, we are legally required to provide the requested information.
Insurance Billing – Information necessary to process claims may be shared with your insurance provider.
With Your Written/Verbal Consent – If you sign a Release of Information form or verbally allow permission, your clinician may communicate with the specified person or organization.
Clinical Supervision – Client cases may be reviewed during supervision to ensure high-quality care. These discussions do not include identifying details.
Mandated Reporting – All clinicians are mandated reporters. If there is suspicion of abuse, neglect, or other harm, we are required to notify the Department of Children and Families.
AVIV, LLC Grievance Policy
AVIV, LLC is committed to conducting all educational activities in full compliance with applicable ethical and professional standards, including the Ethical Principles of Psychologists of the American Psychological Association, the ethics principles of the National Board for Certified Counselors, and the requirements of the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. AVIV, LLC also adheres to all applicable jurisdictional laws governing social work ethics, conduct, and professional values.
AVIV, LLC maintains a strict commitment to non-discrimination in promotional activities, program content, and the treatment of all program participants.
Oversight of compliance with these standards is the responsibility of the Clinical Director in consultation with the Continuing Education (CE) Social Work Consultant. Together they monitor and assess compliance of AVIV’s Continuing Education programs with ASWB ACE requirements and applicable professional and legal standards.
While AVIV, LLC works to ensure fair treatment for all participants and to anticipate potential concerns, issues may occasionally arise that require review or resolution. The following procedure outlines how grievances are addressed.
Filing a Grievance
If a grievance arises related to an AVIV, LLC Continuing Education program, presenter, or process, the complainant should notify the Clinical Director as soon as possible, either by phone or in writing, so the concern may be addressed in a timely manner.
If the grievance involves a social worker or relates specifically to social work continuing education requirements, the Clinical Director and the CE Social Work Consultant will review the complaint together. The written response will document the complaint and the involvement of the Social Work CE Consultant in the review and resolution process.
Review and Resolution
If an immediate resolution is not possible, the Clinical Director, and when applicable the CE Social Work Consultant, will present the complaint to the General Partners within one month. All reasonable efforts will be made to maintain the confidentiality of the complainant.
The General Partners will review the matter, determine an appropriate response, and recommend action if necessary. The decision and any resulting actions will be communicated directly to the complainant.
Examples of potential resolutions may include:
Communicating feedback or concerns to presenters
Adjusting future educational programming
Modifying course content or logistics
Providing alternative training opportunities when appropriate
Documentation
Confidential records of all grievances, including the complaint, review process, and outcome, will be maintained by the Clinical Director in secure files.
All written complaints and AVIV’s responses are also reported to the Association of Social Work Boards as part of the ACE approval renewal process.
